Abstract
Hoya asthma is an occupational asthma among workers in oyster manufacturing factories in Japan, which usually develops after exposure to body fluids of sea-squirts. The prevalence of area in Japan is reported to be about seven percent in recent
years.
We experienced a case of occupational asthma who had worked in a mussel manufacturing factory for 5 years. He had suffered from cough, sputum, dyspnea, and rhinitis symptoms since one year ago. His symptoms were aggravated after exposure to
sea-squirts.
Total eosinophil count was 621/§§ and total IgE-PRIST was 280 IU/ml. He showed positive responses on allergy skin prick test with common inhalant allergens(can fur 3¡¿3/24¡¿16, Chironomus plumosus 2¡¿2/26¡¿18, Tokunagayusurika akamusi
3¡¿3/25¡¿15(mm))
and with sea-squirt extracts9home-made extracts from Styela plicata 7¡¿4/18¡¿11, Hoya extracts of Torii Co. 2¡¿2/4¡¿3(mm)). IgE-RAST to d2, e1, I73 were class 0. The bronchoprovocation test with sea-squirt extracts showed dual asthmatic response.
PC20
methacholine levels before ant after the bronchoprovocation test with sea squirt extracts(day 1, day 5, day 8) were 14mg/ml, 0.62mg/ml, 2.3mg/ml, and 2.2mg/ml, respectively.
We recommended complete avoidence, and immunotherapy was tried with Hoya extracts from Torii Co. His symptoms were much improved with anti-asthmatic medications and immunotherapy.
In conclusion, an occupational asthma with exposure to sea-squirts(Styela plicata) was noted in a patient working at a mussel manufacturing factory in Korea.¡¿
